Your Rights as a Patient
You have certain rights and responsibilities while you are a patient at Rex Healthcare that include controlling decisions about your healthcare and receiving accurate information about your health. A detailed list of your rights will be provided to you at the time of your admission. For more information about patients' rights, click here.
Advance Directives
Advance Directives are sets of directions you give about the health care you want if you ever lose the ability to make decisions for yourself. These forms can be completed before your visit. In North Carolina, there are three types of Advance Directives.

Healthcare Power of Attorney (POA)
This form lets you name someone to make decisions about your healthcare if you become unable to decide for yourself.
