If you ever lose the ability to make decisions for yourself, advance directives outline your wishes for your physician and family in regard to your health care.
Rex provides you with a complete advance directives packet that can be downloaded and completed before you arrive for your procedure/treatment.
In North Carolina, there are three types of advance directives:
- A healthcare power of attorney (POA) lets you name someone to make decisions about your healthcare if you become unable to decide for yourself.
- A living will informs Rex and your family of your healthcare decisions should you become unable to do so.
- Advance instructions provide instructions and preferences regarding mental health treatment.
