Medical Social Work
Medical social workers assist patients in gaining access to community resources, educate patients about long-term care options and provide financial counseling. Coordinating with the rest of the care team, our highly experienced and knowledgeable social workers ensure that patients are aware of and have access to the resources necessary to address their health needs.
Home Health Aides
Home health aides provide personal care under the supervision of a nurse or therapist. In conjunction with an appropriate skilled service, we can arrange the service of a home health aide to meet your specific personal care need.
Home IV therapy is provided by highly trained nurses in the patient's home, as ordered by the physician. These nurses educate patients and caregivers on self administration of the IV medication, care of the infusion site, precautions and side effects. They perform laboratory draws if needed.
Nutritional Services – Our services include education by a registered dietitian for new diabetics, tube feedings, cardiac Heart Healthy Diet, and specialty diets ordered by physicians.
Our respiratory disease, COPD Clinical Pathway program provides a team of nurses, physical therapists and dieticians to assess, monitor and educate patients in disease process, medication management and self-help tools. The goal is to decrease the need for re-admission to the hospital and emergency room visits.
Heart Failure Program
This Clinical Pathway program provides a team of nurses, physical therapists and dieticians to assess, monitor and educate patients. It includes self-help tools for the disease process, medication management, weight control, recognition of symptoms of fluid retention and cardiac failure. The goal is to decrease the need for re-admission to the hospital and emergency room visits.
Rex Telehealth Services helps you become educated about, and engaged in, your self care while staying connected with your professional care team. Telehealth is particularly appropriate for patients with congestive heart failure, COPD and/or hypertension. Our staff of trained clinicians provides daily home monitoring of blood pressure, oxygen level and weight. Regular reports can be forwarded to your physician, ensuring a complete circle of care.
