Short-Term/Post-Acute Rehabilitation Services

Our short-term, post acute rehabilitation program is designed to help patients get back on their feet and regain their independence by combining comprehensive exercise, physical and occupational therapy. This unit is equipped with 34 short-term beds and is specifically designed for those individuals that are discharged from the hospital who are not quite ready to return homes. The rehabilitation programs available are intended to provide patients with both education and assistance to allow for maximum independence. A team of rehabilitation specialists that includes your doctor, nurse, social worker, physical and occupational therapists and a speech therapist will design a treatment program to address your specific needs. Once you are ready to begin, your daily schedule will include exercises and activities of daily living. Our team will meet each week to evaluate your progress and make any necessary adjustments to your rehabilitation program. When it is time for you to return home we will assist you with all areas of this transition and work with you to help ensure you are in a comfortable and safe environment.

A fall, stroke, or brief illness can deprive us of our customary feeling of independence and security. It is important for patients to know that there is help and relief available at Rex Rehabilitation and Nursing Care Center of Apex. Our programs are designed to improve the physical condition of patients with limited functioning. Education is also available on exercises that you can do once discharged from our facility as well as techniques to help prevent future injuries. We also offer education to family and loved ones on the use of assistive equipment.

  • Physical Therapy
    With the help of Physical Therapists, patients work toward independence, returning to a fully functioning lifestyle as soon as possible. We offer comprehensive evaluation and treatment. Other problems treated include range of motion, flexibility, endurance issues, balance and gait training, strength and body mechanics.
  • Occupational Therapy
    Patients in need of occupational therapy work with trained occupational therapists to master activities of daily living from dressing and feeding to cooking, shopping and writing.
  • Speech Therapy
    Our speech therapists evaluate and treat patients with swallowing problems, cognition problems, language delay and voice articulation.
