If you need care after you leave the hospital, please let us know. We offer several options to assist you. Rex Rehabilitation and Nursing Care Centers in Raleigh and Apex provides short-term rehabilitation, skilled nursing and long-term care. We also offer outpatient rehabilitation and home health care.
