Advance Directives

If you ever lose the ability to make decisions for yourself, advance directives outline your wishes for your physician and family in regard to your health care.

Rex provides you with a complete advance directives packet that can be downloaded and completed before you arrive for your procedure/treatment.

In North Carolina, there are three types of advance directives:

  • A healthcare power of attorney (POA) lets you name someone to make decisions about your healthcare if you become unable to decide for yourself
  • A living will informs Rex and your family of your healthcare decisions should you become unable to do so
  • Advance instructions provide instructions and preferences regarding mental health treatment
