2025 Community Health Needs Assessment

Every three to four years, Wake County conducts a comprehensive community examination thorough the Community Health Needs Assessment (CHNA) process. Working with the UNC Gillings School of Global Public Health, community health needs and resources were determined using information collected from primary data, citizen opinion surveys, community leaders and focus groups, as well as, secondary statistical data.

The 2025 CHNA was a collaborative effort between Duke Raleigh Hospital, UNC Health Rex, WakeMed Health and Hospitals, Wake County Human Services and non-profit partners including Advance Community Health, Delta Dental, NeighborHealth, the Poe Center and RightCare. The CHNA team was led by co-chairs Wake County Commissioner Vickie Adamson and Elaine Loyack of Delta Dental.
