What to expect
You’ll?receive a call to schedule an appointment within two business days after a referral. If you recently received hospital care,?we’ll?try to schedule your visit within a week of discharge. Rely on us to contact your PCP and primary cardiologist to get your medical records.
Bring all your medications and supplements in their original containers to your first appointment. During the appointment, you will:?
- Meet with a heart failure doctor or advanced practice provider and other team members
- Get lab testing, if needed
- Create a personalized care plan
Follow-up care
Your team will follow up with your primary cardiologist or PCP to discuss your care plan. They may ask you to make lifestyle and medication changes,?monitor?your weight, see a pharmacist, enroll in?
