Rex Heart Failure Clinic

At the Rex Heart Failure Clinic, you’ll work with a supportive team of experts from UNC Health Rex and UNC Hospitals. We understand how heart failure impacts your life, and our goal is to help you manage your heart condition symptoms in the comfort of your own home.

We meet with you to talk about your lifestyle and needs, and you'll check in with our team regularly by phone. We’ll share your care plan with your primary care doctor and your primary cardiologist.

You may get a referral for an appointment from your primary cardiologist or primary care provider, or you can call the office to schedule an appointment without a referral. If you don't have a referral, we’ll ask you to share some information, such as any heart tests you’ve had, to help make sure our services are the right fit for your needs.

Our Raleigh office is one of three Rex Heart Failure Clinic locations in central North Carolina and is an extension of the heart transplant and LVAD program at UNC Hospitals.
